PRIMENA GLUKOKORTIKOIDA U PREVENCIJI KOMPLIKACIJA KRUPA KOD DECE

Krup je najčešći uzrok opstrukcije gornjih disajnih puteva kod dece. Najčešći uzrok su virusne infekcije, pre svih virus Parainflueze. Simptomi krupa nastaju iznenada, najčešće noću i uključuju lavežni kašalj, inspiratorni stridor, promuklost i respiratorni distres. Kauzalna terapija ne postoji i na prvom mestu treba umiriti dete i roditelje. Za decu sa krupom lekovi izbora su glukokortikoidi na svim nivoima težine bolesti, a oni se mogu primeniti oralnim, inhalatornim, parenteralnim ili rektalnim putem primene. Pravovremena primena kortikosteroida kod sve dece sa krupom i nebulizovanog epinefrina kod dece sa respiratornim distresom značajno smanjuje učestalost intubacije, hospitalizaciju i ponavljane posete lekaru.
